Train Fever includes the original game plus the free USA DLC. It is a railroad-focused business simulation with procedural content and a city simulation.

The game begins in 1850, when you establish a transport company, build railways and stations, purchase vehicles, and manage lines. It features passenger and freight transport, dynamically simulated urban development, procedurally generated buildings, and no grid-based building restrictions.

The USA DLC adds a complete American game mode, with more than 150 years of railroad history and over 50 realistic American vehicles, including trains, buses, streetcars, and trucks. It also changes the landscape, towns, streets, and environment, and extends in-game mod support.
